New Patient Registration

Please note, If you are already registered as a patient at Queens Medical Centre you do NOT need to register again to use our smartphone app or submit an econsultation / online consultation. If you are not yet a patient please continue below.

Anyone in the UK can register with our Practice. However, only patients who are housebound and live within the Devon area covered in the picture here, are able to receive a home visit.

There are 3 ways to register at the practice:

2. Download a registration form here and either email it to us or post it to us

3. Attend in person and complete a registration form
